Purpose (e.g., abstract): | To enable research findings to inform clinical practice, the NHLBI appointed an Expert Panel to develop clinical practice guidelines that can enhance the ability of people with SCD to receive appropriate care, support primary care and specialty physicians as well as other clinicians in providing that care, and assist clinicians and other health care professionals in dealing with common management issues. Because SCD is not common, most health care professionals are not sufficiently knowledgeable about delivering primary care to SCD patients. Scientific research has identified findings useful for the clinical care of SCD, yet these findings are underutilized. This project addressed these issues by developing national evidence-based guidelines to assist clinicians in caring for patients with SCD. The Expert Panel developed a guideline report through a systematic review process. The report contains evidence summaries and graded recommendations in the areas of routine health maintenance; recognition and treatment of common acute and chronic complications and co-morbidities; and use of hydroxyurea and blood transfusion therapy. It addresses the care of infants, children, adolescents, and adults with SCD. The Expert Panel members were volunteers in primary care, adult and pediatric hematology, psychiatry, transfusion medicine, obstetrics and gynecology, and emergency nursing. The Panel was sponsored by NHLBI and supported by a team from the Mayo Clinic Knowledge and Encounter Research Unit, an independent methodology group that guided literature searches, prepared Evidence Tables, and reviewed and summarized the body of evidence for the Expert Panel’s deliberation. |
